1/9/2023 EnchantaVaughn Approved Behavior: Client seemed positive upon counselorâs arrival to the home for todayâs individual counseling session in which he self reported he was slightly agitated that he had gotten into an argument with his mother today as he returned home from school 1xâs with moderate intensity for 2 minutes. He reported, coming home to immediately being verbally reprimanded by his mother which caused him to respond in a negative manner and say things he didnât really mean. The client had a hard time understanding why his mom was so mad at him. Client was unable to focus in his session following this matter. Behavior: Client communicated feeling bad about his behaviors this past week. Client reported he has had 2 anger outburst this weekend with moderate intensity with each episode lasting for most of the day. Client reported he has also argued with his mother 3-4 days with moderate intensity over minor issues. He reported he feels left out in the home and feels everyone is against him at times. Client reported he gets down on himself at times and act out in a negative manner. He reported he contacted his Dad and requested to stay with him for a few days until he could get himself together and is ready to sit down and share his feelings with his mom. Intervention: QMHP probed client as to his request for todayâs session. QMHP engaged client in discussion regarding his disrespectful behaviors within the home and school. QMHP processed with client the importance of controlling his actions and thinking before speaking. Probing and unconditional positive regard were used in conjunction with warm acceptance to gather clientâs perspective during session. QMHP reinforced the importance of having good listening skills and following instructions at all times. QMHP then modeled and taught client to utilize alternate methods applied by himself to help him follow directions from all authority figures. Intervention: QMHP implemented calming techniques such as âDeep Breathing and Counting to Tenâ tactics into todayâs session to help client with quickly deescalating a possibly aggressive situation. QMHP then motivated client with a inspirational book into todayâs session to help client continue improving his overall behavior. QMHP then processed the therapy exercises, in which he was encouraged to practice âDeep Breathingâ techniques and document his response to activity in his daily journal. QMHP processed with client his negative and defiant behaviors today as he disobeyed orders from his mother and took it upon himself to do whatever he wanted without considering how it affected other members in the household. Response: Client seemed receptive to todayâs session in which, he verbalized understanding of each technique. Client stated that it has been such techniques that have helped him avoid possible verbal and/or physical aggression at home, school and community. Client was rewarded because he showed QMHP that he could follow directions and appropriately express himself without acting out for attention. He responded positively to being modeled and taught to utilize alternate methods applied by himself to help him follow directions from all authority figures. Response: Client seemed to have taken special interest in the âDeep Breathing and Counting to Tenâ tactics by stating they were positive calming techniques that he has used in the past. Client responded positively to being modeled appropriate ways to follow instructions with things such as, following directions at first prompting. Client verbalized that he really enjoyed being motivated by his counselor during the session. Client then processed the therapeutic exercise in which he was encouraged to practice âDeep Breathingâ techniques and document his response to activity in daily journal. After client completed this he asked if he could go outside and play.
